    PROBLEM DESCRIPTION:
    There is a timing window during striped+mirrored
    io processing on MP machines where the hd_freebuf
    list can be corrupted. If this happens the system
    would crash, depending on how the timing works out,
    in any of the disk device drivers or the iodone
    kernel service.

    PROBLEM SUMMARY:
    There is a timing window during striped+mirrored
    io on MP machines where the hd_freebuf list can
    be corrupted, causing the system to crash in
    any of the disk or lvm device drivers or the
    iodone kernel service.
    In order to fix this the MP safe locking was
    changed in the striped and mirrored iodone
    routines of the lvm device driver.

    If any disk in rootvg goes bad then ipl_varyon
    will fail but return SUCCESS in the return code.
    Right after the ipl_varyon we try to mount the
    root filesystem '/' but the mount will fail
    because rootvg is not really varied on. The
    failure of mount will cause rc.boot to hang
    with an led of 0x555.

    TEMPORARY FIX:
    If rootvg is mirrored you can boot from the
    maintenance cds and reduce the failing disk.
    After that you should be able to boot normally.

    PROBLEM CONCLUSION:
    This problem was introduced while trying to fix the
    same problem creating AIX 4.3.2 NIM SPOTs on an AIX
    4.3.3 NIM server. Both types of SPOT creation can
    not be supported. Therefore, this fix returns the
    code to its' original state so that 4.3.3 SPOTs
    can be created on 4.3.3 systems. 4.3.2 SPOTs on
    4.3.3 systems will contain the mkssys/mkserver errors.
    Use the steps listed in the workaround to get around
    this problem.

    PROBLEM CONCLUSION:
    In _set_vp (500 and above) and in _set_curvp (433)
    there is a place where it checks for VP_SLEEP, if set then
    set the application signal mask . Unfortunately there are
    some instances that the VP_SLEEP flag is not being set.
    The solution is to check for the VP_TERMINATE flag instead.
    We know that when this code gets executed, the VP_TERMINATE
    flag will either be set or not. If not, then that means the
    vp is about to be put to sleep.
