The New Hampshire's Department of Health and Human Services mistakenly
released the Social Security numbers and other personal information of
about 9,300 Medicare Part D recipients two weeks ago.

In letters to those affected obtained by The Associated Press, the
department says it is taking steps to make sure the information is not
used illegally. But it urged clients to initiate credit fraud alerts or
freezes on their accounts.
