Did you find the solution?
I have a similar problem with an Atmel CryptoMemory card (T=0).
Works fine on SuSE Linux with pcsc-lite v1.1.1 and on Windows XP but on MacOSX
1.02 it fails in SCardTransmit() with SCARD_E_NOT_TRANSACTED.
