I'm also using the kernel 2.6.16 (CentOS) and I've found a similar problem with my smart card reader ( SCR333 of SCM Microsystems ), using ifdhandler of openct-project:
readerfactory.c:1130:RFInitiailizeReader() Open Port 200000 Failed
First think I've installed the driver supplied by SCM Microsystem, but this driver has given to me only PROBLEMS. So, with command modprobe, I've REMOVED the module scr333 created by the driver installation from the kernel and I've installed the CCID driver (https://alioth.debian.org/download.php/1563/ccid-1.0.1.tar.gz), compliant with a lot of smart card readers, I removed the folder scr3xxx....bundle from the folder driver of pcsc and now all is ok!
