If the problem also occurs on Windows I would suspect a hardware problem.

I do not see anything interesting in the log. The card becomes mute at
some time.
If you use a USB hub then try without the hub.
Also try to connect the flash driver on a different USB controller.
Any kernel messages? See the output of the dmesg command.
