behavior with Apache versions prior to 1.3.27.
PHSS_27477:
Apache Inside Admin Web Server may exhibit
unexpected behavior.
PHSS_24527:
1) VirtualVault account administration supports password
aging and other strong password controls, but there is
no means to administer it. There are also other strong
login and password controls like time of day restrictions
supported on the underlying OS, but not by the
VirtualVault.
2) A user is not forced to reset his password after another
user administratively changes it.
3) There is no way to set password triviality checks via
the HTTP VirtualVault Administration Interface.
PHSS_24212:
1) Nameserver Address entered by the user through
administration interface is not being validated.
2) VirtualVault does not have any command or GUI
utility available to select or assign an account's
privileges. This means that using the mkacct command
there is no option to set the u_syspriv and u_basepriv
fields of the prpwd file.
3) VirtualVault administrator accounts cannot be created
with a name containing an underscore character.
4) The mkacct program produces unexpected output under
certain conditions.
PHSS_23943:
Alarm creation does not prevent alarm names with
a colon in them.
PHSS_24038:
Apache Inside Web Server may exhibit unexpected behavior.
Defect Description:
PHSS_28111:
Inside Admin server may exhibit unexpected
behavior with Apache versions prior to 1.3.27.
Resolution:
Migrated Apache version for the inside admin
server from 1.3.19 to 1.3.27.
PHSS_27477:
Apache Inside Web Server may exhibit
unexpected behavior.
Resolution:
Corrected the Apache Inside Admin Server to
function as expected.
PHSS_24527:
The underlying OS support for these features was never
enabled in the VirtualVault Administration interface.
Resolution:
1) All strong password and login controls supported by the
underlying VirtualVault operating system will now be
enforced by the application layer. They will also be
configurable from the administration interface.
2) After creating a new user via vaultconfig or the "Create
Account" interface, or after changing the password of
another user via the "Modify Account" interface, the real
user will be forced to reset his password on first login
to the VirtualVault administration interface.
3) The "Modify Account Defaults" screen will now be used to
manipulate whether password triviality checks are
enforced on a systemwide basis.
PHSS_24212:
1) The Nameserver IP address entered by the user through
the administration interface is not being validated.
Anything entered by the user is taken as is and
written into the /etc/resolv.conf file.
2) The mkacct command does not provide any method to set the
u_syspriv and u_basepriv fields of the prpwd file. So,
any new user who gets created will always receive the
default privileges from the system default file. The
only method that is available now to change this is to
edit the prpwd file of the newly created account.
3) The login name checking code(in both mkacct and
vaultconfig) was incorrectly disallowing names that
contained a '_' character within them (like "vv_adm").
4) The function handling the -h option was not allocating
enough memory to hold the path string.
Resolution:
1) The IP address entered for Nameserver is validated
before writing into the /etc/resolv.conf file.
2) Two new options -k and -b have been added to the mkacct
command. The account administrator will now have an
option of specifying the kernel authorizations and base
privileges for the newly created account.
3) The legal_login() and validUserName functions will now
accept '_' as a valid character for login names.
4) The checkhome() function will now allocate the right
amount of memory required to hold the path.
PHSS_23943:

1. Back up your system before installing a patch.
2. Login as root.
3. Copy the patch to the /tmp directory.
4. Move to the /tmp directory and unshar the patch:
cd /tmp
sh PHSS_28111
5. Run swinstall to install the patch:
swinstall -x autoreboot=true -x patch_match_target=true \
-s /tmp/PHSS_28111.depot
By default swinstall will archive the original software in
/var/adm/sw/save/PHSS_28111. If you do not wish to retain a
copy of the original software, include the patch_save_files
option in the swinstall command above:

changed to add the NEC vendor identifiers. After the patch
is applied, the output of ioscan(1M) shows the software
state of "CLAIMED" and the hardware type of "INTERFACE" for
the PCI PIC, System Address Controller, and PCI-X PIC
devices.

system IO scan every time the monitor(s) are launched or
restarted causing scsi lbolts or repetitive PLOGIs.

When an invalid entry is present in the Page De-allocation
Table (PDT) then the STM memory daemon, memlogd, does not
attempt to interrogate Single Bit Errors. The logging
mechanism, memlog, will not be created by memlogd daemon.

configuration of the memory on the system.

lbolts on third party tape libraries whenever run, which in
turn causes problems with a customer's applications writing
to the tape libraries.

channel fabric) were detected by a fibre channel switch
from fibre channel HBAs.
2. DTS JAGae54232 and JAGae43017
STM memlogd daemon does not attempt to interrogate the
memory for Single Bit Errors, instead it goes in an
infinite loop trying to retrieve the information from
the PDT. So the memlog activity log file is not created.
Also, the memory configuration is not reported after
the STM memory info tool is run, instead the tool returns
after logging a message in the activity log.

STM hardware monitors, armmon and/or fc60mon, can perform a
system IO scan every time the monitor(s) are launched or
restarted causing scsi lbolts or repetitive PLOGIs.
2. DTS JAGae54232 and JAGae43017
When an invalid entry is present in the Page De-allocation
Table (PDT) then the STM memory daemon, memlogd, does not
attempt to interrogate Single Bit Errors. The logging
mechanism, memlog, will not be created by memlogd daemon.
Also, the STM memory information tool does not display the
configuration of the memory on the system.

The EMS dm_ses_enclosure monitor would restart every 2
minutes due to a SIGSEGV error and there are two distinct
symptoms as such,
a) dm_ses_enclosure dies immediately, and also leaves
device files /var/tmp/rscsi*. This is the case for a
configuration with HPUX 11.0 or 11.11 and an A6491A.
The aplsrv log would show:

This IO scan problem with arrmon and/or fc60mon causes scsi
lbolts on third party tape libraries whenever run, which in
turn causes problems with a customer's applications writing
to the tape libraries.
In another case, repetitive PLOGIs (port login to fibre
channel fabric) were detected by a fibre channel switch
from fibre channel HBAs.
2. DTS JAGae54232 and JAGae43017
STM memlogd daemon does not attempt to interrogate the
memory for Single Bit Errors, instead it goes in an
infinite loop trying to retrieve the information from
the PDT. So the memlog activity log file is not created.
Also, the memory configuration is not reported after
the STM memory info tool is run, instead the tool returns
after logging a message in the activity log.

The dm_ses_enclosure monitor was exiting with a SIGSEGV
error on DS2300 type of disk arrays when adding the second
array controller path to the list. This was due to Buffer

STM hardware monitors, armmon and/or fc60mon, can perform a
system IO scan every time the monitor(s) are launched or
restarted causing scsi lbolts or repetitive PLOGIs.
2. DTS JAGae54232 and JAGae43017
When an invalid entry is present in the Page De-allocation
Table (PDT) then the STM memory daemon, memlogd, does not
attempt to interrogate Single Bit Errors. The logging
mechanism, memlog, will not be created by memlogd daemon.
Also, the STM memory information tool does not display the
configuration of the memory on the system.
PHSS_27809:
1. DTS JAGae32316
The EMS dm_ses_enclosure monitor would restart every 2
minutes due to a SIGSEGV error and there are two distinct
symptoms as such,
a) dm_ses_enclosure dies immediately, and also leaves
device files /var/tmp/rscsi*. This is the case for a
configuration with HPUX 11.0 or 11.11 and an A6491A.
The aplsrv log would show:

minutes). This was seen on HPUX 11.00 with a A5236A and an
A6255A.

2. DTS JAGad91084
SCSI array disk errors would be sent to EMS monitor disk_em
by default rather than the SCSI disk monitor.
PHSS_27525:
1. DTS JAGae33104
IO errors are received by diagnostic diaglogd daemon from
the kernel driver diag2, and are sent to the proper Event
Management System (EMS) hardware monitors, but the data is
in a different format than what the EMS hardware monitors
are expecting, therefore the monitors can't generate an
event for the error log.
2. DTS JAGae29247
A file descriptor leak which will occur when running the
disk_em or dm_ses_enclosure EMS hardware monitor.
Restart messages from disk_em or dm_ses_enclosure are
logged every 6 hours to /var/opt/resmon/log/api.log on an
HPUX 11.11 server which supports OLA/R (On-Line Addition
And Replacement).

EMFILE (24) errno returned from the catopen system call
indicates the per-process descriptor table is full.
Possible Causes/Recommended Action:
Increase the system configuration parameter that
controls the maximum number of open files for a process
(maxfiles, maxfiles_lim).

This message repeats every 6 hours. The maxfiles and
maxfiles_lim have standard values on the system (60,1200).
3. DTS JAGae30052
Either disk_em or dm_ses_enclosure EMS hardware monitor
will die with a segmentation violation upon startup. This
is an intermittent problem which is not reproduceable or
cannot be replicated.
It was found and reported on one particular system and the
fix was verified on the same system. The segmentation
leaks are not visable, but can occur if either disk_em or
dm_ses_enclosure is running from STM version A.31.00
installation.
4. DTS JAGae33138
Due to an incompatibility between the iCOD (Instant
Capacity On Demand) software and STM CPU library, the iCOD
software will not work correctly. Exact erratic behaviour is
unknown.
PHSS_27167:
1. DTS JAGae26804
The labels of memory extender were identified incorrectly
on N-Class and L-Class systems when viewing the Support Tool
Manager (STM) info tool log file after running memory info
tool.
2. DTS JAGae30805
A debug file was erroneously being created on the system.
3. DTS JAGae25418
The Support Tool Manager (STM) daemon cclogd doesn't handle
sending multiple MAC addresses correctly to the GSP. It
works correctly if there's only a single MAC address.
4. DTS JAGae25419
On several PDC_PAT_CHASSIS capable machines like rp8400,
the chained chassis codes from the cclogd daemon are
incorrectly thought to be orphan chassis codes which causes
erroneous events to be generated by the EMS monitor
dm_chassis.
5. DTS JAGae28998
Power-off events are not being reported in event.log, using
the stape driver via the STM hardware monitor daemon
dm_stape.
Defect Description:
PHSS_28526:
1. DTS JAGae50172
This IO scan problem with arrmon and/or fc60mon causes scsi
lbolts on third party tape libraries whenever run, which in
turn causes problems with a customer's applications writing
to the tape libraries.
In another case, repetitive PLOGIs (port login to fibre
channel fabric) were detected by a fibre channel switch
from fibre channel HBAs.
2. DTS JAGae54232 and JAGae43017
STM memlogd daemon does not attempt to interrogate the
memory for Single Bit Errors, instead it goes in an
infinite loop trying to retrieve the information from
the PDT. So the memlog activity log file is not created.
Also, the memory configuration is not reported after
the STM memory info tool is run, instead the tool returns
after logging a message in the activity log.
PHSS_27809:
1. DTS JAGae32316
The dm_ses_enclosure monitor was exiting with a SIGSEGV
error on DS2300 type of disk arrays when adding the second
array controller path to the list. This was due to Buffer
overflow in the tool library.
2. DTS JAGad91084
The Support Tool Manager scsi library routine identifying
the driver name was looking for the typo 'disk30' instead
of 'disc30'.
PHSS_27525:
1. DTS JAGae33104
The data that diaglogd daemon uses to send error log
information to the EMS hardware monitors was changed and
was different than what the EMS monitors are expecting.
2. DTS JAGae29247
This problem will occur ONLY under the following conditions:
1) Support Tool Manager version A.31.00 (June 2002 release)
2) File /dev/olar exists on the system. (On-Line Addition
And Replacement compliant)
3) Either disk_em or dm_ses_enclosure EMS monitor is
running.
If all the above conditions are true, then the /dev/config
device file gets opened repeatedly without being closed.
This file leak will cause the system runs out of file
descriptors.
The fix that this patch includes is to close the file
descriptor to /dev/config after each open.
3. DTS JAGae30052
This problem will occur ONLY under the following conditions:
1) STM version A.31.00 (June 2002 release)
2) File /dev/olar exists on the system. (On-Line Addition
And Replacement compliant)
3) Either disk_em or dm_ses_enclosure EMS monitor is
running.
If all the above conditions are true, then the segmentation
violation may occur when either of these monitors start.
The memory leaks will not be visable.
The code fix that this patch includes is to not free memory
that was never allocated (caused segmentation violation)
and free memory that was allocated (memory leak).
4. DTS JAGae33138
iCOD tools depend on current status of the CPUs. The values
defining the status were changed for this (June) release of
STM diagnostics, which rendered the CPU library
incompatible with prior iCOD releases.
PHSS_27167:
1. DTS JAGae26804
The labels of memory extender were identified incorrectly
on N-Class and L-Class systems. This was caused by the
incorrect mappings for translating the extender numbers to
the extender labels in STM memory information tool.
2. DTS JAGae30805
The cclogd daemon creates a debug file:
/var/kgeorge/echo.txt.
This file contains information for debugging purposes only
and should not have been created.
3. DTS JAGae25418
The problem is a misconception to what the output of
the 'lanscan -n' command is. It was originally thought when
tested that the output showed how many MAC addresses on a
single line. The output is actually one MAC address id per
line. Since cclogd piped this output and the output
from 'lanscan -a' to the same tmp file, cclogd would think
there's only one MAC address and then read the next line
and send the value of '2' as the address. This means that
even the one address sent is incorrect.
4. DTS JAGae25419
Cclogd generated a chassis code chain with the hostname
information in ASCII data. The management processor (MP) on
several PDC_PAT_CHASSIS capable machines has a bug where it
can't decode chains properly. When the MP doesn't see the
chassis code encoded field EOM bit set, it thinks it's an
orphan chassis code, generates another chassis code to that
effect and reflects them back to cclogd. Cclogd writes the
chassis codes to a file and when the EMS monitor dm_chassis
sees them it creates erroneous events.
5. DTS JAGae28998
The error code STE_SCSI_DEV_INIT_FAILED was not being
handled by the daemon due to changes in the default error
handling behavior. The code changes required were the
additions of this error code, as well as the inclusion of
this error code to the dm_stape error table.
