Error 462 occurs unexpectedly

I apologize if this is not a well thought out question and will try to
provide as much information as possible. We have just recently released a
new version of our software to the field (second version running under
Windows XP Pro) and we are experiencing some failures with error code 462.

The application runs on a single computer and uses MS SQL Server as the
database engine. There is a mixture of VB 6 and Visual C++ components both
in DLL and EXE form. The application will generally run fine but
experience periodic failures and report back the 462 error. Since it has
only happened in the field (i.e. unsure how to reproduce it in house yet) I
am still gathering more information. The failure occurs either when
accessing an out of process server (C++) or SQL Server (line numbers aren't
granular enough) from a in-process VB component. This code is clearly
exercised many times before the failure occurs quickly followed by similar
failures in 2 other components (probably related but not sure at this
point). It also appears as though the application critically fails and is
then restarted by the user and runs for some time (hours, days, weeks)
before the next failure.

These standalone computers operate in a workgroup (of 1) and are interfaced
to proprietary instruments via NIC card operating at 10Mbs Full duplex with
a private IP address. The instruments do not directly interface to these
COM components but there is a process that handles the communications
protocol and will use these components, but again, there is generally quite
a bit processing that occurs before a failure occurs.

Any help concerning issues around error 462 would be greatly appreciated.
