This is probably the most common cause of blue screen on WTS 4.0.
The root of the issue is that the NT4 drivers are not multi-user aware. This
happens very commonly with Lexmark and HP drivers, but its really not a
vendor specific issue. HP and Lexmark both released driver updates that
should be Terminal Server compatible.
Below are a couple of pieces of mail I received a while back, one about
Lexmark and one about HP:

As everyone knows there has been a ton of issues with the Lexmark printer
drivers crashing Terminal Server. I have just been informed that there are
new drivers that address the issue. At this time the Readme that comes with
the download does not talk about the Terminal Server issue but Lexmark is
working on getting the file updated. To the best of our knowledge the new
drivers works per the customer I had been work with who got Lexmark involved
and confidence is high at Lexmark that they have this resolved. So please
save this link for future issues that come across your desk so we can get
our customers taken care of. If you have written any SO's/KB's on this
matter please update them. Also please pass this information on to the team
members at the other sites.

The updated LaserJet 5 / LaserJet 6P NT 4.0 driver has been released to the
web site. This driver resolves the stop 50 errors in NT Terminal server.
Customers can download it from:
ftp://ftp.hp.com/pub/printers/software/lj304en.exe
This executable also contains drivers for the LJ4000, LJ5000, and LJ2100
printers.
Unfortunately, if customers start at our home page www.hp.com, and follow
the links to download a LJ5 or LJ6P driver, they will still get the old,
broken driver. I will work with the web folks to get this fixed. In the mean
time, have customers download the driver from the link above, or follow the
links to download the LJ4000 PCL 6 driver, as this will get them the same
"bundle" of printer drivers.
