You'll get more respondents on a dovecot list. You're running two
things that are not in the default install. plus fetchmail. So it's
kinda mutant.

Me? for the dup's I'd look at the MTA logs to see if it's coming IN
10-15 times (fetchmail is likely in that case).

200 messages/day is not large volume. 10k/day is the start of that
category.
