RFC 821 (http://www.faqs.org/rfcs/rfc821.html) is your friend:

section 4.2.1. REPLY CODES BY FUNCTION GROUPS

A 554 error can occur during the DATA phase. Obviously saving the message
(to a database) failed, thus the whole transaction failed. The mail has not
been delivered.
