Adding another box is by far the easier way of increasing availability.
It is entirely possibly to have two Postfix servers with identical
configuration files. K-net.dk uses such a setup:

mailout1.k-net.dk. IN A 192.38.208.86
mailout1.k-net.dk. IN A 192.38.208.81

mailin1.k-net.dk. IN A 130.225.71.227
mailin1.k-net.dk. IN A 130.225.71.226

mailin2.k-net.dk. IN A 192.38.208.81
mailin2.k-net.dk. IN A 192.38.208.86

Outgoing mail is sent to mailout1 while mailin1 and mailin2 are used for
the MX records (the difference between mailin1 and mailin2 is the network
connection). So far (after around two years, I think) only power failures
have made both servers unavailable at the same time. Only a few aliases
differ between the server setups (email to news gateways, for example).

It can be done, of course, but why? Then you lose the potential gain in
availability and the extra performance (assuming you have a need for it),
and you gain nothing, as far as I can tell, by separating incoming mail
from outgoing mail.
