I'll have to have a look.
In principle the only "patch" I include to smtpd now is one to allow for
multiline EHLO greetings, which the standard smtpd doesn't provide. The
solution I used, pending a better one, wasn't 100% "nice" and that's why
Wietse didn't adopt it within the standard sources.

Are you sure that this doesn't correspond with the session transcript?
If not I'll check the patches I made. It's been some time since I've
touched this code and when I did get postmaster notices I don't remember
having seen the type of session transcript you are reporting.
If the bug is mine (I'm not doubting it is) I'll need to see what to do to
fix it.
