    I had, but so had they. They did a sasldblistusers. Maybe the realm is
    wrong?

    saslpasswd -p -u `postconf -h myhostname` -a smtpd -c

    I create a userid by the above means, as suggested in the readme, and I have

    smtpd_sasl_local_domain = $myhostname

    in main.cf.

    Say, p dont think, do you have all your realms matching?
