So few? We have 10 or more running.

master.cf, line for cleanup says: maxproc=0, meaning unlimited.

You seem to have lots of header_ and or body_checks.

From empirical experience, your Exchange admin needs to put down the
crack pipe and open a window to disperse the fumes.
