In Tony's case this can be done in his real-time SMTP proxy, with the
backend SMTP rejecting the client. He is not using fetchmail, his mail
is pushed on demand by his ISP using SMTP.
Suitable code in the proxy is quite simple, the key thing to ensure is
that the "XCLIENT ADDR=<extracted_ip>" command is only used for messages
that arrive from the ISP.
