SASL auth - fatal: per-session SASL client initialization

I am trying to enable SASL authentication in the
postfix smtp client using Cyrus SASL. According to
the logs, the transport map, user and password
database lookup, and smtp ELHO command all occur
correctly. Then I get:
:starting new SASL client
:fatal: per-session SASL client initialization

Here is my /usr/local/sasl/smtpd.conf file:
