Postfix header/body checks aren't fine-grained enough to detect an image
file with no text, this is a job for SpamAssassin.

One choice is to only accept hotmail messages from hotmail
servers. Examples are in the archives.
Or maybe use some RBLs to reject mail from known bad hosts (definition of
"bad" is left up to each RBL operator; check their listing policy to see if
you agree with them).
