Forget the HOWTOs, look at the Courier-imap documentation. Yes, it
requires Maildir, but no, it does not require MySQL.

Not necessary unless you want to do this.

This is easy. See the home_mailbox parameter in postconf(5). OTOH if
you're fine with mbox, you might look at Dovecot, a POP/IMAP server
which supports both formats.
