250 recipient ok (in reply to DATA command) bounces

I am getting a lot of bounces from multiple domains with the above error
message. I realize that the data response is supposed to be 354, but this
doesn't solve the problem of mail getting bounced. Is there anything that I
can do to troubleshoot the problem? This problem is intermittent. Sometimes I
can send to yahoo.com, sometimes it bounces. Any ideas would be greatly
appreciated.

My system is running:
RedHat AS/3
Postfix 2.2.4
Amavisd-new 2.3.2
