H du Plooy had the SAME PROBLEM in 2005! Maybe he knows the answer:

Ralph, my apologies, I completely missed your reply. Yes, I do run the
MX. Sadly I didn't get to figure out what was wrong because the hard
disc on the box died and they opted to replace the whole box (was a
Pentium classic)

Next thing you know, someone will nail 95 patches to the door, then
register postfixreformation.org. Hmmm, no, on further thought,
religious zeal is better suited to other MTA's anyway. As are the 95
patches.
