Btw, I wonder whether an incompatibility with sendmail regarding the
interpretation of hdridx parameter in insheader milter call is noted?

(in my opinion the concept of not providing the new Received header field
to milters is the source of all evil, nevertheless retaining compatibility
may be desired)
