You didn't show enough information to help. By the way, this is more a
question for the cyrus-sasl Mailinglist. Postfix contacts via libsasl2 the
saslauthd Daemon which shows the correct values in the Debug-Output. The next
step would be to check the configuration of pam_mysql, mysql and of course
the Database-Layout and values.
