You left out essential information:

- The Postfix message with the TYPE (mysql? ldap?) and NAME of the
table, and the reason WHY the lookup failed.

- The date stamp, process name and process identifier

Do the following command:

$ egrep 'warning|error|fatal|panic' /var/log/maillog

and report COMPLETE logfile records around this mail delivery
attempt. If necessary, you can replace user names by UUU and domains
by DDD.
