I just went through what sounds like the exact same thing.
What was wrong on this end was, the Internet Mail Service was getting bogged
down. Searching Google got me a couple of entries that identified my problem
exactly. However, the solution they gave (install SP4, which was already
done but I did again anyway) didn't help.
What I found was a message (a 175MB message!) stuck in the Internet Mail
Service queue as "inbound message waiting for conversion". The message was
addressed to an address we no longer had. The message could not be deleted
via the delete button. Nor could it be deleted using Mdbvu32.exe (see
Q165505). The message didn't even show up using this method.
I ended up having to delete the queue.dat file (see Q197792).
